Medical Weight-Loss Program

Program fee: $99 per 4-week period, automatically renews every 4 weeks unless canceled.

What’s included in the program fee:

  • Initial medical evaluation (in-office or telehealth, as clinically appropriate)
  • Ongoing follow-ups and medical oversight (frequency based on clinical need)
  • Dose management and side-effect support
  • Patient communication through our patient portal (not for urgent or emergency concerns)
  • If indicated, up to one B12 injection administered in office per month
  • Orders for labs or other services, as clinically appropriate

Medication (important):

If a medication is prescribed, it is dispensed and fulfilled by a licensed pharmacy (which
may include a compounding pharmacy or outsourcing facility). Medication availability,
formulation, and pharmacy fulfillment timelines can vary. When applicable, the Practice collects medication payments only as a payment agent for the pharmacy. Medication charges are listed separately, identify the dispensing pharmacy, and are passed through at cost with no markup.

Program fee: $179 per 4-week period, automatically renews every 4 weeks unless canceled.

What’s included in the program fee:

  • Everything in the Medical Weight-Loss Program above
  • Monthly nutrition coaching session (provided by Registered Dietitian)
  • Weekly nutrition check-ins via patient portal messaging (not for urgent or emergency concerns)
  • Lifestyle and accountability support designed to support adherence and tolerability

Program fee: $249 per 4-week period, automatically renews every 4 weeks unless canceled.

  • Monthly nutrition coaching session (provided by Registered Dietitian )
  • Weekly nutrition check-ins via patient portal messaging (not for urgent or emergency concerns)
  • No medications included
  • No injections included

Important Program Information

Important Program Information

  • Self-pay only. Insurance is not accepted.
  • All programs and services are subject to medical evaluation and provider judgment.
  • Month-to-month plans automatically renew every 30 days unless canceled.
  • To avoid the next billing cycle charge, cancellation requests must be received at least three (3) business days before the next scheduled billing date.
  • Prepaid multi-month plans are billed upfront.
  • Telehealth services are provided only to patients physically located in New York (and any other states where the provider is licensed) at the time of the visit.
  • This program is not for emergency care. For emergencies, call 911 or go to the nearest emergency department.
  • Compounded medications are not FDA-approved finished products

Can I start or continue weight-loss medication through telehealth?

In many cases, yes. Depending on your medical history and provider judgment, you may be able to start and continue weight-loss medication through telehealth, with follow-ups, dose adjustments, and ongoing care often handled virtually. In some situations, an in-office visit or lab work may still be needed.

Do I need to come in for my first visit?

Often, no. Many patients can complete their initial consultation virtually and, if clinically appropriate, begin medication after that visit. However, your provider will decide whether telehealth alone is sufficient or if an in-person visit, exam, or additional testing is needed before starting treatment.

Are virtual visits as effective as in-office visits?

For many patients and visit types, telehealth can be a very effective way to receive follow-up care, support, and dose adjustments. There are still situations where an in-office visit is more appropriate, and your provider will recommend that when needed.

Can I switch between telehealth and office visits?

Yes — many patients use a mix of in-office and telehealth visits, depending on their schedule and clinical needs. Your provider will help you choose the right format for each visit.
