PRICING POLICY
Billing & Membership Structure
All programs are self-pay only. Insurance is not accepted.
Month-to-month plans are billed every 30 days and automatically renew every 30 days unless
canceled.
3 month programs are billed upfront at enrollment.
Cancellation Policy
Patients may cancel at any time by providing written notice to: info.tonedmd@gmail.com.
To avoid the next scheduled charge, cancellation requests must be received at least three (3) business days prior to the next scheduled billing date.
For month-to-month plans, cancellation becomes effective at the end of the current billing cycle.
No refunds are issued for services already rendered, visits completed, or amounts already incurred for pharmacy fulfillment that cannot be reversed.
Multi-Month Prepay Plans and Early Cancellation
Multi-month pricing is offered in exchange for a time commitment.
If a patient cancels a prepaid plan early for non-medical reasons, any refund calculation will be based on the standard month-to-month rate for the portion of the plan already used, plus any non-refundable costs already incurred (e.g., completed visits or non-reversible pharmacy fulfillment costs), with any remaining eligible amount refunded, if applicable.
If treatment is discontinued for a medical reason (including side effects, intolerance, contraindications, safety concerns, or provider medical judgment), the Practice will not impose a “discount forfeiture” recalculation, and any eligible refund will be determined consistent with amounts already incurred and services already rendered.
Medical & Corporate Disclosure
All medical services are provided exclusively by a physician-owned professional medical corporation, which is solely responsible for all clinical care, prescriptions, and treatment decisions.
The brand and operations are supported by an MSO that provides administrative and non-clinical services only. The MSO does not provide medical care and does not control medical decision-making.
Clinical Disclosures
Participation does not guarantee eligibility for medications, injections, or specific outcomes.
Individual results vary; no specific outcome is guaranteed.
If a medication is prescribed, it will be based on medical appropriateness and patient safety, including medical history and any required evaluation/testing.
Pharmacy and Medication Fulfillment
If medication is prescribed, it is dispensed and fulfilled by a licensed pharmacy (which may include a compounding pharmacy or outsourcing facility).
Compounded medications are not FDA-approved finished products; the FDA has not reviewed compounded medications for safety, effectiveness, or quality.
Medication availability, formulation, packaging, and fulfillment timelines may vary due to clinical, regulatory, or supply considerations.
Telehealth
Telehealth is not appropriate for every condition or patient. The provider will determine whether telehealth is clinically appropriate and may recommend an in-person visit, exam, labs, or testing when needed.
Telehealth services are provided only to patients physically located in a state where the provider is licensed at the time of the visit.
Not Emergency Care
This program does not provide emergency medical care. For emergencies, call 911 or go to the nearest emergency department.
Patient portal messages and any nutrition check-in messages are not continuously monitored and are not intended for urgent or emergency concerns.
Payment Issues
If a payment fails, services may be paused until payment is resolved.
If a payment is reversed and remains unpaid after notice and an opportunity to cure, services may be paused or discontinued.
Right to Discontinue Services
The Practice reserves the right to refuse or discontinue services if participation is deemed unsafe, inappropriate, non-compliant, or not in the patient’s best medical interest.
Privacy
Patient information is handled in accordance with applicable federal and state privacy laws, including HIPAA.
