Most of the conversation about medical weight loss today centers on one category of medications: GLP-1 receptor agonists. Semaglutide (the active ingredient in Ozempic and Wegovy) and tirzepatide (Mounjaro, Zepbound) have produced results that genuinely changed what medically supervised weight loss looks like. But they are not the right tool for everyone — and at TonedMD, we think clearly about that distinction before recommending any protocol.

How GLP-1 Medications Work
GLP-1 (glucagon-like peptide-1) receptor agonists mimic a naturally occurring gut hormone that is released after eating. They work through several mechanisms at once: signaling the brain that you are full, slowing the rate at which food leaves your stomach, and reducing the reward response that drives overeating. The result, for most patients, is a significant and sustained reduction in appetite.
The medications TonedMD uses — semaglutide and tirzepatide — are administered as once-weekly subcutaneous injections, typically self-administered at home after your provider teaches you the technique. Doses are started low and increased gradually over several weeks to minimize side effects.
The clinical evidence behind these medications is substantial. Patients in large trials lost an average of 15–20% of their body weight over 12–16 months with semaglutide, and results with tirzepatide have been even stronger in head-to-head comparisons. For patients who qualify, GLP-1 medications are often the most effective tool available.
Who Qualifies for GLP-1 Treatment
General candidacy guidelines are well established: most healthy adults with a BMI of 27 or above (or 25 and above with a weight-related condition such as high blood pressure, elevated cholesterol, or pre-diabetes) who have struggled to lose weight through diet and exercise alone are potential candidates. There are also contraindications — a personal or family history of certain thyroid conditions or pancreatic issues, for example — that your TonedMD provider will review with you during your consultation.

Non-GLP-1 Options at TonedMD
Not every patient wants to use a GLP-1 medication, and not every patient needs one. TonedMD offers a range of non-GLP-1 approaches that can stand alone or complement a GLP-1 plan:
- Peptide therapy — Certain peptides support fat metabolism and lean muscle preservation. We use them for patients focused on body composition rather than scale weight alone.
- B12 and lipotropic (MIC) injections — Methionine, inositol, choline, and B12 work together to support fat metabolism and energy. These injections are a common add-on for patients already on a GLP-1 plan and a first-line option for those who do not qualify for GLP-1 medications.
- Medical-grade nutrition guidance — TonedMD includes nutrition counseling as part of every weight-loss program. For some patients — particularly those with modest goals or strong dietary habits they want to build on — a structured nutrition plan with regular check-ins is the primary tool.
